Screen 4.3 - Caregiver details #43

Need to review the whole screens. We already have the screens in Ionic.

@dianeoliveira @Suj511 I'm working on the caregiver screens. My query is if this list is only about physicians or specialist, why use the term "caregiver"? This term gives a impression of a family member or paid helper who is taking care of a the patient. Just a thought!

  1. Added new text input field : "first name", "Last name", "speciality", "website",
  2. In Ionic app, the label is outside the text field. As per guidelines, Every text field should have a label. Refer ionic documentation to follow (https://ionicframework.com/docs/api/components/input/Input/)
