Hello, to give some news :
We've been monitoring the issue since friday when Ella notified us, and it is a quite evasive one. We added some additional logging to try to figure out why the issue happens, it seems to be an issue with forwarding the grades back to Coursera through LTI. So far all I can tell is that it seems to be happening very rarely, yesterday it only happened once out of more than a thousand score submissions through LTI, but I don't know yet why that happened, the issue is that we cannot reproduce and it's so rare that we cannot do much but wait until it happens again, see the data we got, and try to pinpoint more thanks to that data.

Thank you for the update! Is there any way to help from the TA end? Like when people post about their grades not syncing, is there any way to trace that?
Some additional information from a learner:
Any time I tried to load an App Item with a video accompanying it, it would take a very long time to load and would even put up the message "This is taking longer than expected to load...please wait" and then it would fail and ask if I wanted to "load anyway (for debugging)." I would select the option to load anyway and it would seem to work, but then if there were graded items it would let me do the quiz, but it would not recognize my score until I logged out, closed my browser and logged back in and resubmitted my solutions. It seemed to think I had never submitted them in the first place, which I definitely did.
I did manage to get my scores recognized, but still thought I should report it since I'm pretty sure that's not how it's supposed to be working, ideally.
I'm using Mozilla Firefox 132.0.1 (latest version as of this post) in Pop_OS! 22.04 LTS, I also made sure all my ad blockers were off, if that helps.
Recently been noticing a lot of learners saying that their activities are not being marked as completed, even after they get 100 on the activity. Additionally, people have been saying that they have to submit multiple times before their activity actually gets marked as completed.
Have been replying to people and telling them to make sure they open the activity directly through coursera that way they are definitely linked to their account, but have already received a reply back saying they do that and it still isn't working. That post is linked here.
