Generated picture is not out #92

What steps will reproduce the problem?
1.If you try to open my website (http://vcoffice.um.edu.my), sometimes
there is no output picture.

What is the expected output? What do you see instead?
If it is opened using IE, pictures column is replaced by cross. In Firefox,
the picture column is blank.

Please provide any additional information below.
The output picture only occurs, if I clear the cache (clear recent history
in Firefox, delete browsing history in IE) first.
