Add Coveralls and Dialyzer #19

Some dialyzer cleanup and add a couple tests for obviously uncovered code paths.

Would close #17 and #16

@geowa4 I couldn't enable the Coveralls integration because I don't have permissions. I can help set it up if you want. Here's a link to the report for my fork: https://coveralls.io/github/dantswain/medera

Heh. Tests failing b/c Coveralls isn't set up.
