Add distinction between `osm2pgsql --append` and `osm2pgsql-replication` #295

Details

Currently both osm2pgsql --append and osm2pgsql-replication show up under osm.pgosm_flex.osm2pgsql_mode = 'append'. The difference between the two is important and should be repored.
