I combined what I wrote in the competing risk vignette with what I presented at the CSSC 2017. I removed the comparisons with Cox regression and Fine-Gray, as they are a better fit for the manuscript on competing risks.
The formatting is still strange, and I haven't quite figured how to make nice tables. But these formatting issues are probably left for when we will have an almost complete manuscript.
