Creates a form for Speakers, Sponsors, and Venue owners to enter their information. Also fixes the google map on the network tab to only be shown when searching for the categories we want to map Venues and Code-Schools

Commented out import statements in src/components/forms-page/index.js will be used later when button functionality is added in our next pr
