I have the same issue, if you find a fix for this, or if anyone has found one please tell.
You've listed 92 different mods, this is not helpful to deduce where the issue is occurring. Please find out what mod exactly causes the conflict and crash.
it says it right there, it's "cofh_code", which is a library that other mods depend on. see what mod depends on it and try updating both the library and the mods it depends on, or regress to an older ver. if possible?
The problem is 100% Optifine. For some reason, it imagines that there is something wrong with the namespace (there isn't), and crashes.
Vanilla Minecraft has no problem with the shader. No other shader mod has any problem with the shader. It's entirely Optifine freaking out over nothing and choosing to crash. We've simply told people to stop using it, as there is absolutely no reason for this check/crash to exist.
i solved this problem thru this like 1/2hour later lmao yeah optifine sucks right now, probably the worst fps maxxer there is right now
I recently came across this issue, im guessing theres no chance of a fix as its been so long?
