Improve accessibility of upload-error page #111

A blind user reported that it was difficult to determine when a book had
failed to upload properly.  The error message came far down in the page, in
document order.

Review the layout of this page and make it more accessible, at least by
putting a report of the upload status at the top of the document.

It would also be nice to report an 'upload successful' message on the
library page, again, above the library listing.
