Implement support for ops:switch

The developers of MathType have created a sample ePub (attached) showing their 
"best practices" 
for math in ePub. The file contains [1] MathML, [2] SVG and [3] PNG versions of 
an equation. 

The interesting thing is they use ops:switch to choose between the 
alternatives, depending on what 
the reading system supports.

Right now, all three versions show up in Bookworm. It would be cool if Bookworm 
implemented 
ops:switch.

Thanks, I completely agree.  It's actually the same issue as #147, but I'm 
happy to have a real-world test case.  
I'll merge that one into this.
