Table of contents does not list level-5 sections (I.2.3.4.5), making those undiscoverable via navigation

Add all levels to table of contents
put level-5 sections (and deeper) to level-4 documents
Add YAML front matter to level-4 documents with navigation links to its immediate "child" documents
