determine how to parse the POSTed data from Content-Type HTTP header if available
return 400 error if anno wasn't created (content-type mismatch or bad jsonld or ...)
create honors HTTP Accept header if present

Addresses: #36; #20 (except want to beef up tests for obtaining media type from Accept header for #create), also part of #113 (tho not html piece)

waiting for tests for Accept headers for #create action before merging, and possibly squashing commits.
