Represent HSA-Id and introduce attribute-set and entity category #113

There are some use cases where the HSA-Id attribute is relevant to be used as a primary identity. We should reference the SAMBI attribute specification (https://www.sambi.se/wordpress/wp-content/uploads/2019/05/Sambi_Attributspecifikation_1.5.pdf) and clarify the use of HSA-Id within the Swedish eID Framework.

The use cases for releasing an HSA-ID without a personal identity number is not clear to me. Perhaps we should settle for just define how a HSA-ID attribute should be represented at this point ...
