Feature/questionnaire hygiene

As a user I want to see some questions for 'hygiene' as well as answer possibilities So that I know how how much time I spend for menstruation-hygiene

Unfortunately I had to build my formComponent from scratch because it didn't make sense at all. So this US became bigger than intended. I did not adjust the former questionnaires yet so it doesn't become even bigger (will do so in a future US)

Also I made some changes to the US while coding. See here what is different:

The form (2. question) accepts answer for: from 1 to 30 minutes --> 1 / 15 / 30 minutes
Calculation of index adjusted to new answer-possibilities
The outputs also calculate minutes into days and years
Change of wording for infobox
