Please describe your problem:

I get to the point where it asks for a username and password.  I enter my 
information and click next.  I get a message that says "Query failed!"

Your version of ezStats: 0.91

Hi!

It seems, you've entered the table prefix of your Joomla during installation. 
This is not correct! ezStats needs own database tables. So enter "ezstats2_" as 
prefix f.e., then it should work.
