Copy: We will now ask you a series of questions to gather evidence of your eligibility for support and an advocate under the Care Act.

The application will be broken up into three sections:

Eligibility Gathering evidence of your eligibility against the outcomes outlined in the Care Act

Wellness Checking the extent to which your wellbeing is negatively affected by the obstacles outlined in the eligibility section of the application

Advocacy Gathering evidence about your need for an advocate to help with the process of receiving support under the Care Act

Take your time answering the questions and don’t worry if there are sections which you do not see as relevant to you.

Answer to the best of your ability and expand as much as necessary in the text input boxes. Remember that your own needs may differ from the examples that we have provided - please elaborate as necessary.

Gathering evidence of your right to a Care Act assessment and eligibility for support against the outcomes outlined in the Care Act.

Wellbeing Checking the extent to which your wellbeing is negatively affected if you do not get the support you need to meet the outcomes outlined in the Care Act.

Advocacy Gathering evidence about your need for an advocate to help with the process of receiving support under the Care Act.
