Fix Changelog format and link to changelog website #715

Hi, I'm the creator of Openchangelog and a big fan of time.

I created a changelog website for time using Openchangelog which you can check out at https://time-rs.openchangelog.com. It currently looks a bit weird since your CHANGELOG.md doesn't follow keep a changelog completely.
I fixed these minor formatting issues to comply with the keep a changelog format.

Would love to have this linked in the README to make it easier for users to stay updated with time's changes.

Aside from the one extraneous #, this is not something I am interested in. I particularly have no interest in promoting a service that I've never heard of when a link to the changelog itself would more than suffice (though I do not want this either). With regard to the formatting, the format is based on keep a changelog, not identical to it. The brackets presumably serve a purpose for markdown by indicating a link, which is not present even in your change.
