Fixes #132

Text change to the deduped eholdings header to "Online".
Additions of external link icon to all eholdings links.
Addition of call number, public note, and materials specification note fields to the link display.
Fixed the ghost empty eholdings that were displaying in some cases.
Better CSS for eholdings.
