I don't think I do - Or I don't understand this. Do you have an example?
There is Egon Miller. He married Eve Smith and takes Egon Smith as married name. He then gets the Smith patriarch. But the Smith patriarch is not related (bluts verwandt) with Egon.
He then gets the Smith patriarch.
I'm afraid I cannot reproduce this. I either need a link to an online tree that shows this, or a (partial) GEDCOM file, to be able to follow up on this.

The listet patriarch is Miller.
'Miller' is listed for Debbie Smith (as it should be), that has nothing to do with the married names. See here.
I notice the "Include/Exclude individuals with “Smith” as a married name" switch doesn't have any effect, but that is a separate issue.
Everything seems to be ok. I will try to remember suspicious pages better in future.
I think you should not follow the Marriage Name for finding the patriarchs.
