Fix changelog and update README with VP badges etc #36

Renamed 'Changelog' to HISTORY.md and fixed up the headings the best I could, (considering several releases are missing git tags etc.) Then regenerated CHANGELOG.md using master branch of gcg.

Added badges to README.md and added LICENSE file for github's license detection to work.
