Unable to confirm pipeline task #323

I started a PR merge pipeline task (https://whosonfirst.mapzen.com/boundaryissues/pipeline/275/), which got to "Phase set to confirm" without error. However, there is no response when I try to click on the "Confirm" button to merge the changes to master.

The Slackbot notification doesn't work either; I get the following error when trying to confirm though Slack:

Darn – that didn't work. Feel free to give it another go.

Other links to the pipeline and git diff seem to work as expected.

@dphiffer - I haven't encountered this recently, the pipeline tool and Slackbot have worked as expected on recent runs.
