improve kindle rendering

I have basic support for converting the epub to a mobi file using make roads-and-bridges.mobi, but the result leaves a bit to be desired in the Amazon Kindle for Mac app. I haven't tested on a physical Kindle yet.

A few notable issues to be fixed:

• [ ] no cover image in main library view
• list styling seems to get dropped
  • [ ] table of contents is rendered with item numbers
  • [ ] callouts are being rendered with bullets
• [ ] callout background not rendered at all

sorry, I'd promised I'd do this. :) I can do this weekend for sure, maybe earlier, and let you know what I see. I was going to use a free epub -> mobi converter unless you have the file handy already.

Ok, checked it out on my Kindle Paperwhite! I clicked through the first couple of chapters. Things I noticed (most of which you already noted above):

• No cover image ("Roads and Bridges / Unknown") in main library view
• Sort of unnecessary title page, following the original cover page (guessing there's a reason why you added this - could be replaced with the copyright page?)
• "Open up your phone..." title page is missing
• Section titles ("Foreword", "Executive Summary") don't appear (I'm guessing bc it's white text, and the colored background isn't loading, so changing to black text and removing background would fix)
• Pull quotes appear gray (I think they're red in the color version), might be easier to read if they were just black
• Callout boxes (ex. "Some tools that a software company uses are:") are rendered with bullets or numbers (ex. "Four freedoms" box), and missing their shaded background. Might be best just to drop list styling and indent?

Overall though, this is just minor styling, and I find the main text to be quite readable. It's exciting to be able to read it on a physical Kindle!
