Blacklist issues

25k sats bounty for a PR and closure of this issue The blacklist is working but with a few issues.

• When a pubkey is permabanned with a word from the permaban list and manually removed from the config and the config is save the server restarts as expected but the pubkey is still considered permabanned until closing and restarting the server fully ctrl + c
• max_temp_bans does not seem to work at all. If someone makes notes whose content contains temp ban words more than the max number, they should be added to the permaban list.

Server still requires hard restart to remove a blacklisted pubkey but temporary bans and adding to the permanent list after exceeds is working as intended.
