One thing to ask, was that Google account made just for this test or was it being used before?
Ah, it was being used before. However, it hasn't happened again since the first post.
Sorry, something I forgot to add. The only times it seems to have happened is when I've returned to long-open windows – eg, those left open overnight with a link to Youtube – rather than quitting and restarting between each session.
Ah, hmm.
YouTube was marking accounts server-side so a clean account is preferred for the test but I will be investigating the issue in depth as I have gotten some more reports including a video which shows a weird behavior which shouldn't be happening.

How are you making accounts without sms mobile number requirement?
Very occasional detection at browser start and first video watched – this is purely to give information to the extension author so please don't join this thread unless your sandboxed conditions are the same:
• Mac Chrome Version 126.0.6478.183 (Official Build) (x86_64) – I use Brave as my daily driver, so Chrome is used for Youtube only • Latest version of Fadblock with no other extensions loaded in Chrome • A dedicated Google account for Youtube only.
As this is only happening very occasionally (perhaps once a week at most), signing out of the account, clearing all Chrome data and signing back in solves the issue with minimum fuss.
