h3 labelss not apperaing on TOC #847

What steps will reproduce the problem?
1.mark up a chapter has Heading 3
2.Display TOC
3.H3 labels entries not appearing in TOC 
...

What is the expected output? What do you see instead?
I expect nested Toc entries

What version of the product are you using? On what operating system?
0.4.0.beta 3

Here is an example of H2 tags - all over the place.. ie why don't they appear 
in the order they are in the document?

The Table of Contents sidebar shows the items as they appear in the NCX file, 
*NOT* as they appear in the content files. Your NCX file in the epub has the 
items all jumbled up, and they appear in this order in the TOC sidebar.

Clicking the "generate toc from headings" button rewrites the NCX using the 
headings in the content files.

The Old Sigil never worked like this? 
How do I get around it? i.e. reorder the TOC to the order in Epub file?

Sorry I dont understand.
perhaps it has become too complicated for me.
How does this get fixed.

It is the same with quite a few Epubs..

Has no toc.

use Silgil and it inserts a jumbled up Toc..

and another file which has the chapters in order and when Sigil adds them to 
toc it puts them in a strange order.
