update validation rules for latest version of element guidance #232

This updates the validation rules to reflect the latest Data Elements guidance published on 11-19-2015.

The changes are in the four "rules" files that specify expected column names, data types, and field lengths for each element in the four files ingested by the broker. These files drive the simple data validations currently performed by the broker.
