Bug Description: The 'Date' field on the Assurances tab is not validating the information correctly (e.g., I entered a date in the future -- year 3000). The system should not allow entering future dates.

TODO: @EStriegel what should the requirement be? Today's date only? past date allowed? future date and past date doesn't make sense. Your call.
