Links to citations in the first section of the rule #392

In the "Comment headers" section, most of the internal links are caught properly:

However, some are not linked because we do not capture formatting after a certain level in the document:

It looks a little broken now since some work and some don't (for no reason the user can discern). Can we link as close as possible to those sections?

Moving this out of the milestone as it won't be simple and isn't a user request.
