Create hand-off documentation

Extended Description As an ASAP team member, I would like the structure for handoff documentation to exist, so I can iterate on the documentation with a story-based workflow.

Acceptance Criteria

• [x] In a format available to all stakeholders to read and provide and feedback
• [x] Format supports iterative, story-based additions

I'd like to try to write the handoff documentation in a form that also serves as a public reference of what project this has succeeded in producing.
