One thing I'd like to suggest is a short "What do we do with this?" blurb (or link) that explains our methodology. Some simple language along the lines of, "We combine these responses to see which sites need our help. Every X weeks, the top Y sites with the most complaints are selected for review by the Digital Coalition."
Too many interactions with government are of the "sending a form into space" variety-- It's important for users to know that they're being heard.
