I just ran into a feedback survey at the bottom of a USA.gov page! Here are screenshots:

I also ran into a tiny positive feedback widget on sites.usa.gov FAQ pages, such as this page:

This seems to be a WordPress widget called "Q and A Focus Plus".

Nice! Made me jump over to HC.gov to see what they have. If you see here: https://www.healthcare.gov/coverage/dental-coverage/

It's interesting that this HealthCare.gov form doesn't have a prominently-noted OMB control number, unlike the USA.gov and HHS.gov forms.

The recently launched betaFEC has launched with a brilliant feedback mechanism. All fields optional, sticky to footer, submits issues openly and automatically to GitHub.
