Header & Footer states

In order to complete https://github.com/18F/myusa/issues/698, https://github.com/18F/myusa/issues/711, https://github.com/18F/myusa/issues/672, we need to document exactly what's in the header and footer at each point. Also @juliaelman has questions that need to be answered.

1. Logged out of MyUSA

Header

• About: landing page
• For Developers: developer docs
• Contact Us: contact modal
• Sign in or Create an Account: the 2.1 Sign in or Create an Account page (also described in #736)

• Privacy: https://staging.my.usa.gov/legal#privacy-policy

2. Signed into MyUSA

Header

• My Profile + email: https://staging.my.usa.gov/profile#
• Log Out: logs visitor out of MyUSA

Footer

3. Signing in from integrator

Header

• MyUSA + lock icon + Integrator logo (integrator logo must be constrained to the same height as the MyUSA logo)

Footer

• same as above

4. Integrator doesn’t have a logo

• MyUSA + lock icon + Integrator app name (plain text, in the same place as where the Integrator logo was in the previous screen)

Footer

For "Sign in or Create and Account" there is a reference to "2.1_SignInCreateAccount.png" Which screen is this for?
