Setup New Payroll: Update fields for Steps 1 and 2 #85

Step 1:

We should auto-populate the prime contractor field, since only prime contractors can initiate this view and we can pull company name based on the user's identity.

Step 2:

--At the top of the list, add a field for identifying the contracting officer (so the rest of the system knows who to send reports to) --For brevity, consider changing field names to company name, email, phone # --Add a field after company name for a primary point of contact --Add fields that captures the start and end date for each subcontractor added

@acolter per our conversation around sprint 3 priorities, let's see how people react to the idea of capturing start and end dates for associated subcontractors and discuss to what extent, if at all, the system should leverage this information.
