Add elements of service design and lean product #4

I'm proposing some changes to incorporate elements of service design and lean product thinking to the first description of the overall approach. I think the way it's described now is more "feature factory" rather than building towards specific outcomes. I also wanted to tease that these efforts should include potential changes to how the service operates, rather than just the software itself.

@kaitlin, we did a lot of user testing of this handbook (especially in June and July, hence the enormous changes since your PR), and a clear response from our target audience is that we needed to keep everything really simple. Whenever possible, we have eliminated nuance or gray area, since this isn't a guide for people who are going to be doing anything in the realm of agile or lean product thinking or service design, but for people who are at least two steps removed from people who will be doing all of that. Your suggestions are good and right, but not right for this audience, as revealed by our user research. (We had to kill a lot of our own beloved nuance, too.) I appreciate it, though!
