I don't know if is the rignt place to ask fort it, but it is possible to have the form UI design in QGIS as HTML in the response in GetFeatureInfo ?

No, it's not linked to Py-QGIS-Server. But Lizmap implements it in two ways. Either with "HTML Maptip" popup (legacy QGIS popup in the documentation) and then click "Copy the Drag&drop form layout" or "Form" popup. https://docs.lizmap.com/current/en/publish/configuration/popup.html#form-popup
