Feature: Create Spring Boot Starter #35

PR to create Spring Boot Starter for Fluent Forms and incorporate FormsFeeder Reverse Proxy code into that starter.

This will enable Spring Boot apps to leverage Fluent Forms more easily. This is intended to make custom Spring Boot apps easy enough that it will reduce the need for FormsFeeder.
