Reviewed all 45 cases.
Only 1 was seriously in error such that it should "throw an exception". Code name "CT340". In this case, the Volume of Study of CT did not include the entire thorax, but thorax was truncated in several slices to the right side (of patient). In these slices, the lung was essencially in continuation with outside body space, and eliminated. This case is in reality a "badly" performed exam, and in many institutions might be refused to be reviewed by the radiologist because part of the lung is not observable and may contain pathology which would then be missed.
Conclusion: This type of situation should be rejected by system.

All other cases contained small pockets of air either in the gut or in sub-cutaneous tissue.
For the moment, these extra volumes could be considered acceptable.
A 3D volume continous algorithm or size-based air space elimination would likely eliminate these spaces without subtracting from the lung space.
All cases contained trachea.
