Replace list-tables with csv-tables, for all tables defining File Header and Data Columns.
Now tables are external csv files, which makes them easier to edit. All csv files are stored in a dedicated tables folder. Note that csv files are rendered as table in GitHub, but are not embedded in the corresponding rst files. In ReadTheDocs, this is displayed properly.
