Improved DX12 state tracking for next ReShade version and preview alpha clearing:

Add option to clear alpha channel in the preview of render targets to avoid ImGui blending it completely away. Currently only for dx9-dx11
Improve state tracking by restoring descriptor heaps in DX12. Requires presumably ReShade 5.9.3+
Some general refactoring
