Rework moderation #18

Moderation is piggybacking on the status field at the moment. It shouldn't be.

Add a separate moderation status field (to the core Issue model):
- unmoderated (initial status if mod policy in effect)
- public (initial status if no other policy)
- hidden (hidden by a moderator)

When not authorized, the API mustn't show issues that are not public.
