Remove questionnair #6

Questionnaires aren't any more available to user. But they are still in codebase. It may be a good idea to remove them.

This was a very popular feature during sicamp, and we spent a lot of time making it. Also, last time I looked, when someone clicks on the donate button, they are first going through these questions

I forgot to update link on main page. Done now.

It is a feature on which we really spent some good time but it turns out that right now it is not necessary. Probably it will resurrect in some another form but by now it should die (it is not my decision).
