Donation form duplication

These two forms are almost the same: donations/new.html and case_donations/new.html. Can we merge them?

case_donation -> donation is created for a specific case, for Doamna Galina or Petru Popa or anyone else. donation -> unassigned donation, simply person wants to donate blood

can we have one generic file (ex. _donation in layouts) which will be included in both case_donation and donation but with different parameters?
