Outbound calls get disconnected, internal calls work.

What steps will reproduce the problem?
1. entering an outside call number from dial dialog box or click contact to dial
2. click dial
3. pickup phone extension

What is the expected output? What do you see instead?
You should start hearing the call party ringing but you get the asterisk's 
goodbye message and get disconnected

What version of the product are you using? On what operating system?
Asterisk (Ver. 1.4.24) Freepbx 2.8.1.4, outcall ver 1.60, win vista

Please provide any additional information below.
calls to internal extensions works, incoming cid and opening contact in outlook 
works, just outgoing calls to external telephone numbers do not work.  asterisk 
logs show call fails, dumps call.

i've got exactly the same problem.

Outgoing calls internal = OK
Incomming calls outlook = OK
Outgoing calls external = NOK

Windows 7 Outlook 2007 Asterisk 1.4.37
