when i save a question as saved as new, I can't find the original question.
Does the system replace the old question with the new?

When you create a new version of a response domain, the question item will still only save the original version.

In current QDDT the search in QuestionItem is case sensitive.
It should not be case sensitive. It works for categories

I entered a new scale domain called "Sausage-Apple"
I saved 3 major versions and some as work in progress. In timestamp the revisions do not appear when clicking "include revisions"
