[INFRA] Add issue template and proposal information

Using OHBM hackathon as a template. I still need to actually modify the template.

What other information do we want to include in this repository? Pinging @llevitis and @jessicabartley.

I think we need to figure out what we want projects to look like. Will each project have its own repository with issues before the project proposal? I think that level of organization is unlikely, but something like an attached analysis plan could be realistic.

I'm not sure about what specifically should go in the projects repo but I agree asking students to have their own repo before proposing a project is a bit too much. Do we like how the OHBM Hackathon did it here?

Also @llevitis did you say you had access to the labels HBM Hackathon used to categorize/organize projects? Those might be helpful as we think about this.
