This PR:

adds a design first to document how users can edit FHIR data using tabular representation
adds a test first set of tests to illustrate test expectations
linked to user facing documentation here

A little concerned with the deep-merge library being a dependency issue in the future, but also recognize that there aren't many/any libraries that do specialized python dict merging operations that are super well maintained

General architecture/user story seems adequate.

@matthewpeterkort @lbeckman314 Validation exception caused by version drift in fhir.resources dependency, which in turn was caused by this branch not properly based on development branch. Closing this PR, deleting this branch and re-opening on new branch #32
