Added in phpLiteAdmin to directly access the backend of the DB.

Added a questionnaire_response column to the db.
Added the new container to the docker-compose file

@terry77228 can you please take a look and see if this works with your front end code to display html text from the questionnaire_response column to the db. You can Access the admin page at http://localhost:2015/phpliteadmin.php
