Update docs to reflect migration issues around ks0108 library #16

The docs: readme.txt and the doxygen ks0108 migration need a few
updates for transitioning.

They need to indicate how to get new glcd library up if old ks0108 release is 
installed.
- Remove old ks0108?

They should describe where to copy custom fonts & bits files to
make sure that they work with the new library

How to get custom fonts & bitmaps to show up in
the allfonts.h and allbitmaps.h header files.
