Task improvements

This PR implements the following changes:

• Multiple choice task has only 4 possible answers
• Multiple choice task has an optional explanation field
• The Next task button is hidden until submitting of the form
• The question and instruction fields in annotation task have a default value and cannot be edited
• Instructions for two-column tasks are left aligned when doing the task
