Fix aidava:hasPatient domain and range and domains of hasCode and hasBodySite #36

The domains Diagnosis and Procedure for the newly introduced property hasPatient are to be redefined, because they are currently not referring to the proper concept.

The domain Age is to be removed because it should be related to Diagnosis only.

The range Patient has to be added.

The domains Diagnosis and Procedure for the properties hasCode and hasBodySite have to be changed to the sphn concepts.
